The fish oil supplements that millions of American men take each day to cut their risk of heart disease might have a dark side, at least according to a study released today that is sure to generate controversy.

The new study, published today in the Journal of the National Cancer Institute, suggests that men who have higher levels of omega-3 fatty acids in their system face a 43 percent increased risk of developing prostate cancer and a 71 percent increased risk of the high-grade form of the disease.

To determine this, the researchers relied on data from a past study that examined the blood concentrations of omega-3 polyunsaturated fatty acids in 834 men with prostate cancer and 1,393 men without prostate cancer. When they did this, these researchers found an association between high omega-3 levels and the occurrence of prostate cancer.

Whether the increased level of omega-3 in the men with prostate cancer was from supplements or from oily fish in their diet was unclear. But lead study author Dr. Theodore Brasky of Ohio State University Comprehensive Cancer Center said that no matter the source, the findings suggest that men should be wary of getting too much of the nutrient.

“Men will probably want to talk to their doctors, especially those patients who have been recommended increased fish oil intake,” Brasky said. “They should probably moderate their intake of fatty fish and they should avoid fish oil supplements at this time, especially considering that when men are taking fish oil supplementation they are taking [higher doses than they need].”

But some experts say more evidence is needed before men make drastic changes. ABC News Chief Health and Medical Editor Dr. Richard Besser said that the original study from which this data was drawn was not specifically designed to look at the exact relationship between omega-3 fatty acid intake and prostate cancer, so men should think twice before discontinuing them entirely.

“This is not proof that omega 3′s cause prostate cancer,” he said.

If anything, Besser added, the finding should reinforce the notion that better dietary health does not necessarily come in a pill form.

“If you want omega 3′s, get them from food,” he said. “The more we look at supplements, the more untoward consequences we find.”

Diagnosis of Prostate Cancer

The cause of prostate cancer is unknown. It is known, however, that the growth of normal cells and cancerous prostate is stimulated by male hormones, particularly testosterone.

Compared to other cancers, prostate cancer develops relatively slowly. In fact, many men with prostate cancer will not die from the disease, but with the disease. As a man ages, his risk of developing prostate cancer increases. Over 75% of cases are diagnosed in men over 65 years of age.

When a cancerous tumor is small and is located only within the prostate, the cancer is often not detected. The cancer may not cause symptoms and may be too small for a doctor to get it palpable during a routine examination of the prostate. The doctor performs this examination, which receives the name of digital rectal examination (DRE - digital rectal examination), by inserting a finger into the rectum to feel the size and shape of the prostate.

A man can live for many years without finding out they have cancer. As the cancer grows, however, the prostate can eventually squeeze the urethra, which is surrounded by the prostate. Then, symptoms such as difficulty urinating. Generally, this is the first symptom of prostate cancer. (It is important to note, however, that the difficulty in passing urine can be caused by other non-cancerous conditions of the prostate and not always mean that prostate cancer is present).

With or without symptoms, a growing cancer can also start attacking the cells near the prostate. Simultaneously, the cells may be released from cancer and spread to other parts of the body such as the lymph nodes, lungs, and bones, especially the hip bones and lower lumbar region. The most common symptom of this spread is bone pain.

The American Cancer Society (ACS) has developed guidelines to help doctors detect prostate cancer in its early stages. The ACS recently revised these guidelines to reflect new scientific knowledge. The new guidelines recognize that screening for prostate cancer, including a DRE and annual test to measure prostate-specific antigen (PSA - prostate-specific antigen) in the blood, should be offered to the general male population aged greater than or equal to 50 years of age.

Moreover, men with two or more first degree relatives affected by the disease, or those with african-American origin, should start screening for prostate cancer at an earlier age. Although there is still some disagreement on this matter and until there is more scientific evidence, age 45 years of age may be an appropriate time for men with higher risk begin screening.

There are some circumstances in which prostate cancer screening can not be recommended. How prostate cancer can be a cancer that develops slowly, a man with a life expectancy less than 10 years will probably pass away due to some other disease and probably would not benefit from screening and treatment for prostate cancer. For this reason, the new ACS guidelines include guidance for patients that explains the risks and benefits of screening for prostate cancer.

PSA is a substance produced by normal cells and prostate cancer. When prostate cancer is developed or when other prostate disorders are present, the amount of PSA in the blood often increases. The new ACS guidelines advise men with high PSA results showing a doing a biopsy. This will help determine if cancer is actually present.

A PSA test can usually be considered within the normal range when present values between 0 and 4 nanograms per milliliter, sometimes appearing in abbreviated form (ng / ml) in the laboratory report. If the results are within the range above (reported as being greater than 10 ng / ml), certainly doctor may suggest a biopsy.

Sometimes, PSA results are in borderline or gray zone. This occurs when the result is between 4 to 10 ng / ml. The results of the PSA test in this range can be conflicting and not always mean that cancer is present. Certain other conditions, such as benign prostatic hyperplasia (a type of non-cancerous growth of the prostate, also called BPH) and prostatitis (inflammation of the prostate) can cause an abnormal PSA in the test.

If your doctor believes that the rise in PSA is due to benign disease (eg, prostatitis) you may have to wait and repeat the PSA test a few months later and, if necessary, a biopsy later. The new ACS guidelines suggest a biopsy for any man with abnormal DRE results, even if the PSA is normal.

Because of PSA testing in the gray zone may be conflicting, your doctor may advise you to take one or more PSA tests more modern.

The percentage ratio of free PSA / total PSA is a blood test that measures the amount of PSA circulates free (unbound) amount bound in the blood and other blood proteins. If the PSA results are borderline and the percentage ratio of free PSA is low (less than or equal to 10%) then it is more likely that prostate cancer is present. If this is the case, a biopsy may be needed. If the results of the percentage ratio of free PSA are normal, even with a borderline PSA, biopsy is not required.

Another way to examine the PSA involves the adoption of the reference values for PSA specific to different age groups. Higher levels of PSA are normally observed more frequently in men with older ages than at younger men, even without cancer. A range of reference values for PSA specific to different age groups compares the results of the men within the same age group. If PSA levels are high for a man in relation to his own age group, then there is a greater chance that prostate cancer is present.

In men with older ages with results of borderline PSA, this comparison can be more useful than conflicting. As a result, the reference values for PSA specific to different age groups are not routinely adopted.

If your PSA level has already been measured and a TRUS (Transrectal Ultrasound) has also been performed, then it can be determined PSA density (PSAD). To determine which is the PSAD, your doctor will divide the numerical value of serum PSA by size, or prostate volume (the results of TRUS). The chance of getting prostate cancer is higher when the PSAD is high.

Finally, PSA velocity will show how quickly the PSA level increases during a period of time. Two or more PSA tests are often required during the course of several months. Although PSA velocity may be useful in helping your doctor to better interpret the result borderline PSA, in fact she is not used to diagnose prostate cancer. Instead, it is mostly used as a tool to keep track of how their PSA levels are compared for a certain period of time.

Observation is safe, cost-effective, and results in a better quality of life for patients with low-risk, localized prostate cancer, rather than undergoing immediate treatment, says a study carried out at Massachusetts General Hospital and the Dana-Farber Cancer Institute.

The study findings were published in Annals of Internal Medicine.

According to the authors, their statistical models demonstrated that for 70% of men with low-risk prostate cancer at diagnosis "observation is a reasonable and, in some situations, cost-saving alternative to initial treatment."

Julia Hayes, MD, and team believe that their findings confirm that active surveillance and watchful waiting, often referred to as observation, is a reasonable option with low-risk prostate cancer - an option that is underused.

Dr. Hayes said:

"About 70 percent of men in this country have low-risk prostate cancer, and it's estimated that 60 percent of them are treated unnecessarily."

This is a huge number of patients with low-risk prostate cancer who either receive some form of radiation therapy or undergo a radical prostatectomy (their prostate gland is surgically removed), and face the pain and discomfort of treatment, plus the risk of urinary incontinence and erectile dysfunction.

Localized Prostate Cancer - Removal No Better Than Observation - the (low) risk of death over a 12-year period for newly diagnosed patients with low-risk prostate cancer who underwent a radical prostatectomy is the same as for those who opted for observation, according to a clinical trial called PIVOT, which was published online on 19 July in the New England Journal of Medicine.

Dr. Hayes and colleagues looked at a range of scenarios using mathematical models they had created. They concentrated on male seniors aged between 65 and 75 at diagnosis. They also compared estimated costs that are associated with several forms of observation and treatment.

Patients who are advised to undergo active surveillance and chose this option: Undergo a blood test every three months to measure levels of PSA (prostate specific antigenUndergo a rectal examination twice a yearHave a prostate gland biopsy at one year and then every three yearsIf things remain the same - PSA levels stay fairly constant, rectal examinations reveal nothing unusual, and the biopsies results show the prostate cancer continues to be low risk - the "watchful waiting" continues. If at any time tests show that the cancer has become more aggressive, or is more aggressive than originally thought, treatment aimed at curing the disease begins. Hayes said "This approach could also be described as deferred treatment." Patients with low-risk, localized prostate cancer who opt for treatment to cure the disease may undergo: Radical prostatectomy - surgery to remove the prostate gland and some of the tissue around it.IMRT - intensity-modulated radiation therapy. This is a type of 3-dimensional radiation therapy. Computer-generated images are used to show the shape and size of the tumor. Thin radiation beams of different intensities are aimed at the tumor from several angles. With this type of radiation therapy there is less damage to healthy tissue close to the tumor.Brachytherapy - radioactive seed implants are placed within or near to the tumor. Sometimes known as internal radiation therapy or implant radiation therapy.The team calculated and compared the quality-adjusted life expectancy (QALE) for patients undergoing observation versus treatment.

QALE measures both the quality and the quantity of life lived - it is a measure of disease burden. Quantity of life lived is easy to measure. Factors that influence quality of life include having to undergo invasive tests, the impact of the treatment, disease recurrence, and complications.

The investigators also calculated what the lifetime costs are for each strategy. They ranged from $48,699 for a 65-year old man treated with IMRT therapy to $18,302 for a 75 year-old patient who opted for watchful waiting.

For patients newly diagnosed with localized, low-risk prostate cancer, the researchers concluded that: observation is more effective than initial treatmentobservation in some cases is less expensive than initial treatmentobservation yielded 11 months more QALE than brachytherapy (the most effective treatment)observation yielded 11 months more QALE than radical prostatectomy (the least effective treatment)The team admitted that their study made assumptions based on limited research data, but concluded that:

"It appears that active surveillance and watchful waiting are safe alternatives to initial treatment for prostate cancer based on these assumptions. But it's important to emphasize that these decisions are very much a matter of individual choice."

Co-author Philip Kantoff, MD, said "This study delineates the cost benefit of active surveillance as well as watchful waiting - the less aggressive assessment strategy. A previous study by Dr. Hayes and colleagues demonstrated that active surveillance is a reasonable option for men with low-risk disease and associated with a better quality of life. As non-treatment becomes a more accepted option for these patients, selecting those who require less aggressive assessment including biopsy will become important."

A study carried out at the Case Western University School of Medicine in Cleveland, and presented at the American Society for Therapeutic Radiology and Oncology's 49th Annual Meeting, Los Angeles, in 2007, found that prostate cancer patients who undergo brachytherapy have a 50% lower risk of dying compared to those on watchful waiting/active surveillance.

Prostate cancer patients with low risk tumors may safely defer treatment for many years, researchers from the Beth Israel Deaconess Medical Center reported in the Journal of Clinical Oncology (September 2009 issue).

Study co-author Martin Sanda, MD, said "With the advent of PSA [prostate antigen] screening nearly 20 years ago, we started to detect prostate cancers at much earlier stages. Consequently, while PSA testing has enabled us to successfully begin aggressive treatment of high-risk cancers at an earlier stage, it has also resulted in the diagnosis of cancers that are so small they pose no near-term danger and possibly no long-term danger."

Vegetable Fats Reduce Prostate Cancer Death Risk

Prostate cancer patients who replace animal fats and some carbohydrates with vegetable fats have a lower risk of premature death, researchers from the University of California reported in JAMA Internal Medicine.

In the United States alone, nearly 2.5 million men currently live with prostate cancer. Not much is known about how diet may influence prostate cancer progression and death rates, the authors wrote as background information.

Erin L. Richman, Sc.D., and team carried out a study involving 4,577 men from the Health Professional Follow-up Study with non-metastatic prostate cancer between 1986 and 2010. They focused on the patients' dietary fat intake after diagnosis. Every four years, they completed questionnaires which asked how often they ate and drank over 130 different types of foods and drinks.

Over a follow-up period of 8.4 years, 315 men died from prostate cancer and 1,064 died from any cause. Those who replaced 10% of their dietary calorific intake of carbohydrates with vegetable fat had a 29% lower risk of lethal prostate cancer and a 26% lower risk of death from any cause.

The authors wrote:

"In this prospective analysis, vegetable fat intake after diagnosis was associated with a lower risk of lethal prostate cancer and all-cause mortality."

The results of this study go against what many doctors advise their recently diagnosed prostate cancer patients to do, which is "Cut out all fats from your diet". The right advice appears to be to tell the patient to consume more fat, fewer carbohydrates, and to make sure the fats come from plants and not animals.

The authors concluded:

"Overall, our findings support counseling men with prostate cancer to follow a heart-healthy diet in which carbohydrate calories are replaced with unsaturated oils and nuts to reduce the risk of all-cause mortality. ... The potential benefit of vegetable fat consumption for prostate cancer-specific outcomes merits further research."
Dr. Stephen J. Freedland, from Duke University Medical Center, Durham, North Carolina wrote in an invited Commentary:
"Using data from food frequency questionnaires completed every four years during follow-up, they found that men who consumed more vegetable fat had a lower risk of prostate cancer death.

Thus, in the absence of randomized trial data, it is impossible to use these data as 'proof' that vegetable intake lowers prostate cancer risk, and the authors have carefully avoided such statements.

Dr. Freedland says that the only modifiable risk factor associated with prostate cancer death risk is obesity. In other words, if you are diagnosed with prostate cancer, make sure you don't become obese, and if you are, lose weight.

How to avoid becoming or remaining obese remains unclear, although Richman and team's study suggests that switching from animal fats and some carbohydrates to vegetable fats may have some benefit.

Freedland concluded that "Determining whether this benefit is due to reduced consumption of carbohydrates or greater intake of vegetables will require future prospective randomized trials."

Low fat diet with fish oil supplements slow down prostate cancer progression

Scientists from UCLA's Jonsson Comprehensive Cancer Center reported in the American Association for Cancer Research that men who ate a low-fat diet with fish oil supplements had slower progressing prostate cancer compared to those on a traditional Western diet that was high in fat.

The researchers also found that the participants on a low-fat, fish oil supplement diet were able to change the structure of the cell membranes in both healthy and malignant cells in the prostate. They wrote "They had increased levels of omega-3 fatty acids from fish oil and decreased levels of omega-6 fatty acids from corn oil in the cell membranes, which may directly affect the biology of the cells, though further studies are needed."

When blood samples were tested in a test tube, the team found that the growth of prostate cancer cells was much slower among the men on the low-fat, fish oil supplement diet.

Mediterranean diet reduces prostate cancer risk

An article published in UroToday showed that a traditional Cretan Mediterranean diet may help reduce the risk of prostate cancer. A typical diet from Crete, a Greek island in the Mediterranean, is based on plenty of plant foods (vegetables, fruits, wholegrain cereals, legumes and nuts), olive oil as the main source of dietary fat, low red meat consumption, moderate to low consumption of dairy products, moderate to high fish intake, and moderate intake of wine (consumed mainly with meals).

The authors added that the Mediterranean diet has other health benefits and can help protect against cardiovascular and cancer mortality, as well as Parkinson's and Alzheimer's diseases.

Vegan diet plus exercise may halt/reverse prostate cancer progression

Men with early stage prostate cancer who adopted a vegan diet and did plenty of exercise were more likely to stop and even reverse their cancer progression, researchers from the Department of Urology, the University of California, San Francisco, and the Memorial Sloan-Kettering Cancer Center reported in the Journal of Urology (September 2005 issue).

The authors claimed that theirs was the first randomized, controlled trial that demonstrated clearly that lifestyle changes may influence the progression of any type of cancer.

Advanced Prostate Cancer Drug Xofigo Approved By FDA

Xofigo (radium Ra 223 dichloride) has been approved by the US FDA for symptomatic late-stage (metastatic) castration-resistant prostate cancer that has reached bones but not other organs, i.e. with no known visceral metastatic disease.

The Food and Drug Administration (FDA) approved Xifogo under the priority review program, three months ahead of schedule.


According to an online FDA communique published this week, Xofigo is aimed at male patients whose prostate cancer metastasized despite receiving medical or surgical interventions to reduce testosterone levels.


Testosterone, the male sex hormone, stimulates the growth and spread of prostate cancer. Prostate cancer develops in the prostate, a gland in the male reproductive system located below the bladder and in front of the rectum.


Quoting statistics from the NCI (National Cancer Institute), the FDA informs that approximately 238,590 men in the USA will be diagnosed with prostate cancer this years - 29,720 patients will have died of the disease in 2013.


Metastatic prostate cancer commonly affects bone. Approximately 90% of metastatic prostate cancer patients have evidence of bone metastases. When cancer reaches the bone, the patient is much more vulnerable to skeletal events, which are known to be the main cause of morbidity and death in men with castration-resistant prostate cancer.


The FDA was scheduled to make a decision on Xofigo by August 14th, 2013. However, approval came through early because the medications was reviewed under the FDA's priority review program, which allows for a speedier process if the medication appears to provide safe and effective treatment and no alternative therapy is available. Drugs which offer significant improvements compared to currently available options are sometimes processed through the same program.


Richard Pazdur, M.D., director of the Office of Hematology and Oncology Products in the Center for Drug Evaluation and Research, FDA, said:


"Xofigo binds with minerals in the bone to deliver radiation directly to bone tumors, limiting the damage to the surrounding normal tissues. Xofigo is the second prostate cancer drug approved by the FDA in the past year that demonstrates an ability to extend the survival of men with metastatic prostate cancer."

In August 2012, the FDA approved Xtandi (enzalutamide) for patients with metastatic castration-resistant prostate cancer that has recurred or spread, including those who received medical or surgical therapy to reduce testosterone. Enzalutamide was approved to be taken in combination with another cancer drug, docetaxel.


When considering whether to approve Xofigo, FDA experts looked at data from one human study, the Phase III ALSYMPCA trial involving 809 patients with symptomatic castration-resistant prostate cancer that spread to bones but not other organs. Patients were randomly selected into one of two groups - the Xofigo or placebo plus best standard of care group.


The main endpoint of the study was overall survival. A pre-planned interim analysis showed that patients given Xofigo lived for an average of 14 months, compared to 11.2 months in the placebo group. Experts also looked at an exploratory updated analysis which was done later - data confirmed extended overall survival among those in the Xofigo group.


Xofigo may cause the following side effects: swelling of a foot, leg or ankle, nausea, vomiting, and diarrhea. Some patients may develop anemia (low level of red blood cells), lymphocytopenia (low level of lymphocytes), leukopenia (low level of white blood cells) and/or neutropenia (low level of infection-fighting white blood cells).


According to Bayer HealthCare, the commercial production of Xofigo is currently underway, initial doses should be on the shelves within a few weeks. Bayer has worldwide exclusive marketing rights for Xofigo. In the United Sates, Xofigo will be co-promoted by Bayer HealthCare and Algeta US, LLC.


Kemal Malik, MD, member of the Bayer HealthCare Executive Committee and Head of Global Development, said:


"Xofigo shows a favorable safety profile and has the potential to improve patient outcomes in a completely novel way. Radium 223 emits alpha particles that affect cancer cells in bone metastases and may contribute to a survival improvement. This FDA approval will provide prostate cancer patients and the physicians who care for them with a new and innovative treatment option."

North American Principal Investigator for the pivotal trial, Dr. Oliver Sartor, medical director of the Tulane Cancer Center, said "Most men with advanced prostate cancer develop bone metastases, which can be life-threatening. Xofigo has demonstrated an anti-tumor effect on bone metastases and an overall survival effect in prostate cancer, making it an important addition to the treatment of CRPC (castration-resistant prostate cancer) patients."

Xofigo's active ingredient, radium 223 dichloride (radium 223) is an alpha particle-emitting radioactive therapeutic agent which undermines the development and growth of tumors on bone metastases. According to Bayer "Radium 223 mimics calcium and forms complexes with the bone mineral hydroxyapatite at areas of increased bone turnover, such as bone metastases. The high linear energy transfer of alpha emitters may cause double-strand DNA breaks in adjacent cells, resulting in an anti-tumor effect on bone metastases. The alpha particle range from radium 223 is less than 100 micrometers, which may limit damage to the surrounding normal tissue."

EMA (European Medicines Agency) has not yet approved Radium 223, neither has any other non-US regulatory body. Bayer says it has submitted a Marketing Authorization Application to EMA for Radium 2234 in December 2012 for the treatment of castration-resistant prostate cancer.
